Prepare Documents Needed to Enroll

1. PROVIDE ORIGINAL COPY OF CERTIFIED BIRTH CERTIFICATE (REQUIRED) – if unable to provide a birth certificate, the following may also be accepted as proof of date of birth: Affidavit of Parentage, Baptism or Church Certificate, Hospital Certificate, Immigration Documents or Passport.
*At this time we will accept a copy of this document.
2. IMMUNIZATION RECORD
Your child must have an updated immunization record OR an immunization waiver from the county Health Department BEFORE the start of the school year.
3. COPY OF DRIVER’S LICENSE
4. PROOF OF VISION SCREENING, HEALTH APPRAISAL, AND ORAL HEALTH ASSESSMENT (KINDERGARTEN ONLY)
Please provide proof of a vision screen, performed in the last three years. Vision testing can be done at the time of the Physical/Health Appraisal. If the child already wears glasses, a note from the doctor is acceptable.
Assessments
K-First Grade (Brigance) & Second-Eighth Grade (NWEA)
You will be contacted by a FCMA staff member to schedule a time for this assessment once we have dates available.
Once the assessments are complete, you will be contacted to schedule a time to meet with our Head of School, Ms. Stacy.
Pay Preschool Deposit (Preschool Only)
Pay $400 to office staff by mailing a check (payable to Four Corners Montessori Academy) or calling the school to pay over the phone.
This deposit goes towards the overall preschool tuition of $7,770. Tuition is broken down into 9 monthly payments of $818.88
